Choice Home Medical is a LOCAL company with LOCAL employees. We provide incontinence supplies and nutritional drinks through Medicaid. Currently, we accept Healthy U (University of Utah), Health Choice Utah, Molina, New Choices Waiver (for supplemental drinks and wipes), and traditional or fee for service Medicaid. Our goal is to help maintain independence, safety, and comfort wherever you live. We would love to show you how we can improve the ease of your care through lower costs with quality products and service, whatever your needs may be. ***Our office is is not a storefront. We drop ship all supplies. ***We don't carry equipment such as: wheelchairs, cpap machines, nebulizers, braces, etc.
